How It Works
Step 1
Intake / Application Office
Begin at the Capital Area Human Services office. Connect with their team either online, over the phone or in person.

Step 2
Request for Screening
Complete the SUN assessment to determine the Urgency of Need.
Step 3
Person-Centered Planning
Identify the type and amount of assistance needed for daily living, and choose your Support Coordination Agency.
Step 3
Freedom of Choice
Select your provider and receive pre-authorized units to begin services.
Port City Disabilities Services accepts Louisiana Medicaid funding through the NOW, ROW, Supports and Children’s Choice Waivers. Private pay options are also available.
